हेयर ट्रांसप्लांट क्या है?
हेयर लॉस genetically programmed होता है। इसका मुख्य कारण है androgenic alopecia. Usually baldness में family history present होती है. Hormones और कई nutrients की कमी या disease की वजह से genetically programmed हेयर लॉस काफी तेजी से होता है.
- Hereditary - यह हेयर लॉस का प्रमुख कारण है जिसमें baldness ऊम्र के साथ बढ़ता है. इसे androgenic alopecia, male pattern baldness और female pattern baldness कहते हैं.
- Hormonal changes- Pregnancy, menopause और thyroid problem की वजह से जो hormonal changes होते हैं उससे temporary या permanent हेयर लॉस होता है.
- Medical conditions - Patchy हेयर लॉस का प्रमुख कारण alopecia areata है जो एक autoimmune disease है, ringworm infection या trichotillomania जिसे hair pulling disorder कहते उस की वजह से होता है. Psoriasis, diabetes, folliculitis और lupus इन बीमारियों की वजह से भी हेयर लॉस होता हैं.
- Medications -Cancer, arthritis, heart problem, gout और high blood pressure इन बीमारियों के लिये जो दवाइयाीं ली जाती है उसके side effect से हेयर लॉस हो सकता है.
- Stress - Physical stress जैसे सर्जरी , हाई ग्रेड fever और ब्लड लॉस की वजह से excessive hair shedding होते हैं. Psychological stress- Mental stress और anxiety यह भी hair loss का एक कॉमन प्रॉब्लम है.
- Poor nutrition - Zinc और Iron की कमी की वजह से हेयर लॉस होता है. Vit.D3, Vit B12, biotin और selenium की कमी की वजह से भी हेयर लॉस होता है.
- Hairstyle and treatment - Eexcessive hairstyling जिसमे hairs को tightly pull करते हैं उससे जो हेयर लॉस होता है उसे traction alopecia कहते हैं. हॉट ऑयल और हेयर treatment या हेयर straightening जिसमे heat का इस्तेमाल होता है उससे temporary hair loss होता है. कई सारे महिलाओ में अगर इन treatment के बाद scar आते है तो हेयर लॉस permanent होता है.

किन मरीजों को हेयर ट्रांसप्लांट की जरूरत है?
जिन मरीजों में permanent हेयर लॉस या baldness हो गया है, जिनकी उम्र 25 साल से ज्यादा है, जिनमें sufficient donor hairs हैऔर हेयर लॉस active inflammatory disease की वजह से नहीं है तो उन लोगों में हेयर transplant कर सकते हैं.
किसे हेयर ट्रांसप्लांट नहीं करवाना चाहिए?
अगर active inflammatory scarring alopecia है जैसे कि आपके डॉक्टर lichen planopilaris, या frontal fibrosing alopecia यह आपके hair loss का कारण diagnose करते हैं तो इनमें हेयर ट्रांसप्लांट नहीं करना चाहिए. Nonscarring alopecia due to alopecia areata, telogen effluvium, anagen effluvium है तो इसमें hair transplant नहीं करना चाहिए, जिन पेशेंट में donor area limited है और जिनका requirement बहुत ज्यादा है इनमें हेयर ट्रांसप्लांट नहीं करना चाहिए. पेशेंट में diffuse हेयर लॉस है जो donor area को भी involve करता है, जो patients blood thinners drugs पर है जिसे बंद नहीं किया जा सकता और पेशेंटस with unrealistic expectations यह हेयर ट्रांसप्लांट के लिए अच्छे candidates नहीं हैं।
हेयर ट्रांसप्लांट से पहले कौन से टेस्ट करवाना चाहिए?
Hair loss के कारण जानने के लिए हम VIT D3, thyroid function tests, ferritin levels, vit B12 ऐसे टेस्ट करवाते हैं and if required Testosterone, prolactin etc. Surgery fitness के लिए CBC, LFT, RFT, PT/PI/INR, chest xray, ECG टेस्ट भी करवाते हैं।

हेयर ट्रांसप्लांट कैसे किया जाता है?
सबसे पहले हम recipient area जहां पर बालों का प्रत्यारोपण करना है उस जगह की marking करते हैं. फिर local anesthesia दिया जाता है. जितने जरुरी फॉलीकल लगने वाले हैं वह सर के पीछे के area से या beard से या body से निकाले जाते हैं. सामने वाले bald area में hole करने के बाद वहां पर बालों का प्रत्यारोपण किया जाता है. इस प्रकिया को लगभग 6 से 8 घंटे लगते हैं.
हेयर ट्रांसप्लांट तकनीक के विभिन्न प्रकार क्या हैं? यानी हेयर ट्रांसप्लांट के विभिन्न प्रकार क्या हैं?
Commonly hairs को extraction करने या निकालने के FUE (Follicular unit extraction) और FUT (Follicular unit transplantation) इन तरिको का इस्तेमाल किया जाता है. FUE यानी हर एक hair root को individually punch करने के बाद extract किया जाता है. FUT यानी skin की strip निकाली जाती है जिसके बाद individual hair follicle वहां से कट किया जाते हैं. अगर हेयर रुत extract करने के लिये robotic मशीन का इस्तेमाल जैसे Artas hair transplant करते है तो उसे रोबोटिक हेयर ट्रांसप्लांट कहते हैं. Direct instantaneous hair transplant इस technique में hairs FUE technique से extract करते हैं और simultaneously implant करते हैं ताकि hairs बॉडी के बाहर कम से कम समय रहे.
कब किस तकनीक का उपयोग करना चाहिए?
कई बार आप पढ़ते हैं या सुनते हैं कि FUE is better than FUT. Practically देखा जाए तो हर एक technique के pros and cons सोते हैं. जैसे FUE के advantages है scarless सर्जरी है. इसके disadvantages है बहुत ज्यादा ग्राफ्ट का yield नहीं की जा सकती है, यह donor area के ऊपर depend करता है. FUT का advantages है ज्यादा grafts मिलते हैं लेकिन disadvantages है की long scar आता है हालांकि वह हम एक अलग technique से इसे छुपाते हैं. Generally हम अगर 4000 से 4500 तक की follicle की requirement है तो FUE prefer करते हैं और 4500 फॉलीकल से ज्यादा की requirement है तो FUE और FUT का combination करते हैं ताकि maximum grafts implant कर सके।

हेयर ट्रांसप्लांट सर्जरी की तैयारी कैसे करें?
अगर आप smoking करते हो तो 3 हफ्ते पहले इसे बंद कर दिजिये. Alcohol सेवन करते तो कम से कम 1 हफ्ते से बंद कर दिजिये. Minoxidil lotion 10 दिन पहले आप को बंद करना चाहिए. जो भी mineral supplements या blood thinner drugs आप लेते हैं तो इसे 2 हफ्ते पहले बंद कर दिजिये. Hairs को आप बढ़ने दें कटवाएीं नहीं ताकि हमें marking के लिये आसानी होती हो. सर्जरी के पहले दिन शाम को और सर्जरी के दिन सुबह आप betasrub से hair wash किजिये. बालों को सर्जरी के दिन oil मत लगाइए. सर्जरी के दिन open button shirt पहनना और साथ में कोई फ्रेंड या रिश्तेदार या parents होना जरूरी है.
सर्जरी के बाद आपको क्या देखभाल करनी चाहिए?
आपको pain killer और antibiotics prescribe की जाएगी जो आपको तकरीबन 5 दिन तक लेनी पड़ती है.
आपको implanted hairs को scratch नहीं करना चाहिए। सीधे सोना चाहिए ताकि implanted hairs damage न हों। पहले 5 दिन तक normal saline से बालों को गीला रखना चाहिए. Head wash 6th day से स्टारर्ट कर सकते हैं. 3 हफ्ते तक सर्जरीकल cap आपको use करनी पड़ेगी और sunlight से आपको बालों को बचाना चाहिए.

When to expect results?
