सबसे पहले हम यह जान लेते हैं की scars क्या होते हैं और वह क्यों बनते हैं? Scars body का healing response है जो collagen deposition और fibrous connective tissue के द्वारा बनाया जाता है. इसमें कोई regeneration process नहीं होती है इसे आप repair process कह सकते हैं
मतलब शरीर पर हुए जख्म या घाव भरने के लिए जो repair प्रोसेस होती है उससे scars रह जाते हैं।
चलिए हम जान लेते हैं scar से scarless journey कैसे तय करनी चाहिए .हमारे देश में बहुत सारे लोग अपने wounds यानी जख्मों का primary इलाज MBBS,BAMS,BHMS डॉक्टर से करते हैं. इसके कई कारण होते हैं जैसे कि काफी लोगों को यह मालूम नहीं होता कि प्लास्टिक सर्जन आप को सबसे अच्छे scarless treatment दे सकते है बाकी सर्जन के मुकाबले.दूसरा मुख्य कारण है कि availability of a plastic surgeon.तीसरा कारण हम injury के समय पर scar के बारे में ज्यादा नहीं सोचते हैं. चौथा और प्रमुख कारण होता है price बहुत बार हम सोचते हैं कि plastic surgery treatment काफी costly होती है.हालांकि ऐसा नहीं है.चाहे आपको अभी अभी जख्म मिला हो या फिर आपको old scars है जो आपको परेशान करते हैं तो यह blog आपकी बहुत मदद करेगा.
मेरे पास बहुत सारे पेशेंट्स आते हैं जो यह expect करते है कि scar बिल्कुल भी दिखना नहीं चाहिए.
Practically अगर कहा जाए तो यह मुमकिन नही है। कई बार लोग गूगल पर ढूढ कर आते हैं या फिर किसी डॉक्टर का दावा उन्होंने सुना होता है पर सच्चाई यही है फक स्कार को पूरी तरह से नही मिटाया जा सकता। हा अगर हमें कोई मैजिक या जादू आता तो शायद ही मुमकीन था। स्कार्स को पूरी तरह से गायब करना मुमकीन नही है हालाकी बतौर plastic surgeon हम scars को कम करके, स्कीन बेहतर करके एक परफेक्ट स्कार जरूर बना सकते । हमें कोई भी किस्म का मार लगता है या फिर सर्जरी का घाव होता है तो स्कार बनना चालू होता है. स्कार हीलिंग की प्रोसेस सर्जरी के बाद तुरूंत चालू होती है और १ से २ साल तक चलती हैं। Plastic surgeon इस प्रक्रीया को सही तरीके से टाके लगवा कर modify कर सकते हैं। इसके बावजूद भी कई सारी चीजें होती है जिनके ऊपर आप का results निर्भर होता है. जैसे की टाके लेने के बाद आपको कौन सी after care instructions provide की गई। आपकी skin type क्या है। क्या आपका scar infect हो गया था? आपकी skin हीलिंग प्रोसेस स्लो है या फास्ट है। क्या आप स्कार को scratch करते हो। Practically ऐसे पॉसिबल नहीं होता है. लेकिन आजकल गूगल में लोग काफी सारा research करते हैं या किसी डॉक्टर ने बोला रहता है कि scar बिल्कुल ही नहीं आएगा तो आपका इस बारे में knowledge होना जरूरी है.आपकी बॉडी के healing response को silicone based cream,silicone sheet या laser treatment ,steroid इंजेक्शन से alter किया हैं.

Good aftercare advice:
अगर आपने आपके टांके किसी और डॉक्टर से लगा लिए हैं तो आपको जानना जरूरी है कि वह कब निकालने चाहिए. फेस पर 3 से 5 दिन, legs पर 10 दिन, पेट पर 10 से 14 दिन. काफी बार शुरवाती दिनों में scar redddish होते हैं और उसे आगे चलकर black हो जाते हैं. तो एक मेडिकल ग्रेड sunscreen जिसका spf-50 और above हो उसका इस्तेमाल करना चाहिए. इसके साथ moisturizer जिसमें vitamin E , aloe vera content होता है वह आपको use करना चाहिए.
Silicone Gel and Silicon Sheet भी स्कार ट्रीटमेंट में काफी मददतगार है:
Silicone gel या silicon sheet मार लगने के या टांके लेने के 3 हफ्ते बाद आपको चालू करनी चाहिए. 2 मिनट तक दिन में 2 बार आपको Silicone gel से massage करना है. Fingers पर छोटी quantity में silicone gel लेकर उसे scar पर clockwise, anticlockwise,zigzag and irregular pattern से massage कीजिए.
Silicone sheet आपको सही तरीके से scar में लगाना है| दिन कम से कम 22 घंटे आपको लगाकर रखनी है और कम से कम 6 महीने के लिए आपको उसी use करना चाहिए. Cica Care and Epi-Derm जैसी सिलीकॉन शीट्स आसानी से उपलब्ध है।
Silicone gel or sheet हमें क्यों use करनी चाहिए?
जब भी आप का scar बनना चालू होता है तो body में जो fibroblast पेशी होते हैं वह collagen form करते हैं.अगर यह collagen ज्यादा मात्रा और irregular तरीके से तैयार होता है तो हमें elevated scar जिसे hypertrophic scar कहते हैं और keloid तैयार होता है.सिलिकॉन gel या sheet collagen तैयार होने के process को modify करते है. Scar treatment का यह एक महत्वपूर्ण step है जिसे आप ignore ना करें.Long term में इसका आपको काफी फायदा होगा.
Pressure Therapy:
Pressure garment scar maturation में help करते हैं. Ccontinuous pressure की वजह से scar tissue को blood vessels और ऑक्सीजन की सप्लाई कम करता है. Pressure garment burn scar में use करने चाहिए, उससे hypertrophic scar और keloid के पेशेंट को काफी फायदा मिलता है. बाकी उदाहरण में Ear clip ear keloid के लिए ,transparent plastic masks face burn के लिए use करने चाहिए.
Scar Massage:
यह दो तरीकों से काम करता है:
Reflex effect- due to stimulation of peripheral nerve muscle relax होते है ,pain कम होता है और overall sense of well being बढता है.
Mechanical Effect – venous return बढता है, lymphatic drainage increase होता है और muscle fibre के बीच में movements बढ़ जाता है

अभी हम जानने की कोशिश करेंगे इस कार को कब ट्रीट किया जाना चाहीए। इसके लिए आपको चार S के सुत्र को समझना होगा।
पहला s - Site of the scar - Are they located in “high-visibility” areas? क्या आपका स्कार शरीर के बहुत दर्शनी भाग पर है? या फिर वह joints or pressure areas such as the palms or plantar feet पर है.
दूसरा S - Symptoms - Pain यानी दर्द, pruritus यानी itching या खुजली होना, contracture “stretching” अर्थात मसल्स में जकडन का एहसास or hyperesthesia स्कार की जगह ज्यादा सेन्सेटीव महसूस करना।
तिसरा S - Severity of functional impairment - To what degree is there functional impairment of a joint, skin segment, neck, orifice, or digit? अर्थात आपको किस मात्रा में स्कार की वजह से काम में दुर्बलता का एहसास होता है, जैसे कि जॉइंट्स, स्कीन सेगमेंट, गर्दन, शरीर के ओपनिंग वाले हिस्से जैसे मुह, नाक, कान इत्याडि, हाथ-पैर की उंगलिया?
चौथा S - Social stigma - लोग क्या सोचेंगे आपके स्कार को देखकर? खासकर चेहरे के स्कार में यह ज्यादा होता है। लोगों में जाकर घुल-मिल ने से कतराते हैं. कुछ पेशेंट्स अपने scarred area को लेकर चिंतित रहते हैं।
अब हम जानेंगे की Surgical corrections कैसे किए जाते हैं:
अगर आपके scars पुराने हैं और unfavourable location पर है तो हम उसे सर्जरी द्वारा सही जगह पर transfer कर सकते हैं. Scar revision surgery ideally आपके scar तैयार होने 6 से 9 महीने के बाद करनी चाहिए.कई बार हम यह समय 2 महीने तक कम कर सकते हैं. Scar revision surgery में simply अगर आपका scar width में ज्यादा है तो उसे कट करके suture याने टांके लिए जाते हैं और उसे एक plain scar तैयार किया जाता है. इस technique को एक scar excision कहते हैं. जो scar long, straight है और relaxed skin tension line को parallel नहीं है उसे scar repositioning surgery किया जाता है. Generally इसमें z plasty ,w-plasty, geometric broken line scar closure technique का इस्तेमाल होता हैं. Common तरीका यह हैं कि आपका कोई भी long scar हो उसे छोटे छोटे scar में break किया जाता है और irregularize किया जाता है तकि कम noticeable हो.Scar का position में change करके उसे छुपाया जाता है.

Laser and light devices in scar management
Lasers का scar management मैं खास role है
पहला skin की spontaneous improvement को चालना देकर स्कीन की colour tone and texture के सुधार में गति लाना।
दूसरा healing की natural process से जोस्कार बनते हैं उस प्रक्रिया में सुधार लाना
तीसरा high-risk patients में बडे स्कार बनने की possibility को रोकना, prevent करना।
CO2 laser, Erbium yag laser,PDL,KTP&IPL यह lasers commonly use होते हैं. अब हम देखेंगे कौन सा लेजर कब use करना चाहिए
Red Scars:
हर कोई scar शुरुआत में Red होता है यह underlying blood vessels के कारण होता है. जो laser blood vessels को टारगेट करते हैं जैसे कि KTP,IPL,PDL इनका use इस type के scar में करना चाहिए.
Cutter scar /deliberate self harm scar:
यह काफी difficult होते हैं treatment करने के लिए. शुरुआती red scars के लिए vascular laser use कर सकते हैं. अगर redness ठीक होता है तो Erbium या फिर CO2 लेजर repigmentation के लिए use किया जाता है .यह best results तभी दे सकते हैं अगर ट्रीटमेंट injury के तुरंत बाद स्टार्ट की हो.
Acne scars के लिय Erbium tag or RecoSMA or Co2 laser use होता है|
Traumatic scars with tattooing में Q switch laser or Pico laser black discoloration को निकालने में मदद करते हैं|
Scars with PIH अर्थात जो scars black होते हैं उनमें Q switch Nd yag, or q switch Alexandria या फिर Erbium laser का इस्तेमाल होता है|
Hypopigmented scars यानि जो white scars होते हैं उसमें 308 excimer laser repigmentation मैं मदद करता है. 1550 Erbium laser भी काफी मददगार है|
Surgical scars में PDL or KTP for redness,RecoSMA or Erbium or co2 laser for resurfacing सर्जरी के 1 महीने के बाद हमें scar treatment के लिए use कर सकते हैं.
अहम सवाल है कि laser treatment कब चालू करनी चाहिए. यह depend करता है कि आपके scar analysis के ऊपर जैसे redness कितनी है,scar का type क्या है, patients को itching कितनी ज्यादा है? Usually २ से 3 हफ्ते के बाद यह start करनी चाहिए. इसको exception है बर्न scar इसमें थोड़ा लेट start करना चाहिये 6 से 8 हफ्ते के बाद तक की skin अच्छी तरह से heal हो जाए और laser के कोई complications ना हो.
Collagen Induction Therapy:
इस technique में microneedling मशीन से या dermaroller से microscopic injury की जाती है.इस injury की वजह से skin heal होना चालू होता है.

Fat Grafting:
